After discharge from our Home Care, we have the ability to monitor your progress and assist as needed. We touch base bi weekly/monthly to be sure you to continue your journey at an optimum level of health and independence while remaining at home avoiding rehospitalizations. Community Nurse accepts Medicare, most private insurance policies and Medicaid. As a not-for-profit, 501 agency, we provide many necessary but unreimbursed services to patients. To the extent that charitable funds are available, no one who is eligible for services will be turned away. Community offers an array of services from the initial consultation, to home health, hospice, respite care, and grief counseling. Hospice, the final stage of BJC's continuum of care, is a special kind of caring for patients with a life-limiting illness. Services are provided in the comfort of the patient's home or skilled nursing facility. The Hospice staff are sensitive to the physical, psychosocial, emotional and spiritual needs of terminally ill adult and pediatric patients and their families.

Occupational therapists may also make cognitive evaluations along with helping patients relearn day-to-day activities. This includes dressing, bathing, writing, communication, using tools, and so on. HCS relies on community support to close the gap for many of our services. We welcome your help – it will mean so much to our friends & neighbors in need. HCS has its roots in the visiting nurse associations that began locally in 1907. Caregiver Support Group
Education - Participates in identifying and meeting learning needs of self. With assistance, uses patient education materials relevant to patient population. Assesses developmental level of patient and factors affecting ability to learn. Volunteers - Volunteers provide a crucial layer of support to patients and families.

Speech therapy, physical therapy, and occupational therapy services can help the patient regain prior function. Quality Home Health & Hospice Service
These services are referred to as home health care and are considered extended care services prescribed by and under the direction of a VA physician. Community Long Term operates home and community-based waiver programs for persons eligible for nursing home care but who prefer to receive their services in the community. Through case management and an individualized service package, waiver clients can successfully remain at home at a cost to Medicaid that is substantially less than the cost of institutional care. If you or your loved one is homebound due to surgery, illness, or an accident, home health services promote healing at home with the care of skilled nurses and therapists. This allows the patient to be surrounded by family and friends while in a comfortable and familiar setting.
Provides competent clinical nursing care consistent with professional standards. Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a hospital setting. Most reimbursement sources require a prognosis of 6 months or less if the illness runs its normal course. Hospice considers the patient and the family together on their plan of care.
